Develop, assess, and coordinate holistic care management activities for long-term care members
Develop personalized service care plans/service plans and educate members and families/caregivers on services and benefits
Evaluate member needs, resources available, and recommend/facilitate plans for best outcomes
Develop ongoing long-term care plans/service plans and identify providers, specialists, and community resources
Coordinate between member/family/caregivers and care provider team to ensure services are accessible
Provide resource support to members and families/caregivers for various needs (employment, housing, participant direction, independent living, justice, foster care)
Monitor care plans, member status and outcomes and provide recommendations to care plans
Interact with long-term care healthcare providers and partners to ensure member needs are met
Collect, document, and maintain member information and care management activities for compliance with state, federal, and third-party payer regulators
Perform home and/or other site visits to assess member needs and collaborate with healthcare providers and partners
Provide/facilitate education to members and families/caregivers on procedures, provider instructions, service options, referrals, and benefits
Provide feedback to leadership on opportunities to improve quality of care and service delivery
Perform other duties as assigned
Requirements
Florida Residency Required
Must live in West Palm Beach County
comfortable traveling up to 75% within a 30-mile radius
Bilingual English/Spanish Highly Preferred
Case Management exp preferred
Requires a Bachelor's degree and 2 – 4 years of related experience or equivalent experience
