About the role
- Develops, assesses, and facilitates complex care management activities
- Evaluates the needs of the member and recommends care plans
- Identifies barriers to care and provides management interventions
- Coordinates care to ensure members receive adequate services
- Monitors member status and revises care plans as necessary
- Provides education to members and their families/caregivers
Requirements
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 – 4 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
