About the role

Key responsibilities & impact
  • Support care management activities and assigned member teams to ensure provider/partner service delivery and continuity of care/member satisfaction
  • Conduct member outreach by telephone and home visits
  • Document care/service plans and related activities
  • Discuss care plans, next steps, resources, questions, concerns, and ongoing education with members
  • Coordinate care activities with healthcare and community providers, partners, members, and caregivers
  • Respond to member and provider inquiries, requests, and concerns
  • Communicate with care managers, practitioners, and others to facilitate member services
  • Support service assessments and screenings and document member care needs
  • Maintain member records according to state and regulatory requirements and distribute them to providers as needed
  • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers
  • Provide education on available benefits and resources
  • Travel approximately 30% throughout assigned Arkansas service areas to meet members, providers, caregivers, and community partners
  • Perform other assigned duties and comply with all policies and standards

Requirements

What you’ll need
  • High School diploma or GED
  • 1–2 years of related experience
  • Must reside in Arkansas or a border city
  • Experience working with Behavioral Health (BH) and/or Developmental Disabilities (DD) populations, including care coordination, case management, community-based services, or related support programs
  • Reliable transportation
  • Willingness to travel as needed throughout assigned service areas within Arkansas
  • Experience supporting Medicaid populations and conducting field-based member outreach strongly preferred
  • For Arkansas Total Care plan: Bachelor's degree in social science/health-related field OR high school diploma with at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
  • Valid driver's license
  • Child and adult maltreatment check before hire and recurring
  • Drug screen at time of hire and recurring
  • Safety-sensitive position designation in Arkansas
  • Ability to comply with contractual requirements, regulatory guidelines, policies, and standards
