About the role

Key responsibilities & impact

Supervise community and resource coordination activities, including member outreach and connection to local resources
Supervise day-to-day activities of community and resource coordination team members
Assign tasks based on state and contractual requirements and member needs
Collaborate with community/resource coordination and care management teams to ensure effective, appropriate member care
Ensure compliance with state and regulatory guidelines and standards
Educate and provide guidance and resources to address member requests and needs
Evaluate team performance and provide feedback on performance, goals and career milestones
Coach and guide the team to improve community resources, outreach services and member connections
Assist with onboarding, hiring and training team members
Perform other duties as assigned and comply with all policies and standards

Requirements

What you’ll need

Must be authorized to work in the U.S. without the need for employment-based visa sponsorship now or in the future
Must reside in the state of Texas
Bachelor's degree and 4+ years of related experience
Equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position may be accepted
