About the role

Key responsibilities & impact
  • Supervise community and resource coordination activities, including member outreach and connection to local resources
  • Supervise day-to-day activities of community and resource coordination team members
  • Assign tasks based on state and contractual requirements and member needs
  • Collaborate with community/resource coordination and care management teams to ensure effective, appropriate member care
  • Ensure compliance with state and regulatory guidelines and standards
  • Educate and provide guidance and resources to address member requests and needs
  • Evaluate team performance and provide feedback on performance, goals and career milestones
  • Coach and guide the team to improve community resources, outreach services and member connections
  • Assist with onboarding, hiring and training team members
  • Perform other duties as assigned and comply with all policies and standards

Requirements

What you’ll need
  • Must be authorized to work in the U.S. without the need for employment-based visa sponsorship now or in the future
  • Must reside in the state of Texas
  • Bachelor's degree and 4+ years of related experience
  • Equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position may be accepted
