Community Based Coordinator
CareSourceCommunity Based Coordinator supporting members with care coordination. Responsible for implementing person-centered plans, consulting with families, and maintaining service documentation.
About the role
Key responsibilities & impact- Understand and implement person-Centered thinking
- Facilitate the person-Centered planning process
- Assist with in person-centered care training
- Coordinate services and health benefits for members
- Consult and collaborate with other professionals
- Assist with ongoing communication with the internal complex clinical team
- Monitor service delivery and compliance
- Complete psychosocial health care questionnaires and behavioral assessments
- Document care coordination activities and member response in a timely manner
Requirements
What you’ll need- High School Diploma or GED equivalent required
- A minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
- Intermediate proficiency level with Microsoft Office, including Outlook, Word and Excel
- Ability to communicate effectively with a diverse group of individuals
- Knowledge of local, state & federal healthcare laws and regulations & all company policies regarding case management practices
- Must have valid driver’s license, vehicle and verifiable insurance.
