About the role
- Serve as a single point of contact for assessment and service coordination
- Facilitate the person-Centered planning process and coordinate services
- Consult with members, families, and professionals to discuss health care needs
- Monitor service delivery for appropriateness of care
- Complete psychosocial healthcare questionnaires and behavioral assessments
- Maintain current and accurate documentation of contacts and assessments
- Assist in educating members about their condition and care options
- Conduct regular travel for member and provider visits as needed
Requirements
- High School Diploma or GED equivalent required
- A minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
- Intermediate proficiency level with Microsoft Office including Outlook, Word and Excel
- Ability to communicate effectively with a diverse group of individuals
- Knowledge of local, state & federal healthcare laws and regulations & all company policies regarding case management practices
- Must have valid driver’s license, vehicle and verifiable insurance.
