Key responsibilities & impact

Work comprehensively with providers to deliver value-based care management initiatives
Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care
Identify and coordinate community resources with patients
Provide patient education and health literacy on chronic condition management
Perform medication management, identifying potential medication concerns
Assist in ensuring timely delivery of services; Home Health, DME, Home Infusion
Resolve patients' questions and manage referrals and appointment scheduling

Requirements

What you’ll need

At least 3 years of direct patient-facing work experience
Active Multi-State/Compact License (LPN) (NCL) (LVN)
Technical aptitude – Microsoft Office Suite
Excellent written and verbal communication skills
Epic Experience (Plusses)
Bilingual (Plusses)
Additional single state licensures (LPN) (Plusses)
