Intake Coordinator – Licensed Practical Nurse (LPN)

About the role
Key responsibilities & impact- Accepting transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients in our network.
- Resolve patients' questions and create an open dialogue to understand needs.
- Identify and coordinate community resources with patients that would benefit their care.
- Provide patient education and health literacy on the management of chronic conditions.
- Assist with medication management, including identifying potential medication concerns, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services to your patients; Home Health, DME, Home Infusion, and other critical needs.
- This individual must be able to quickly adapt to a fast-paced work environment. This role requires most of your shift on the phone.
Requirements
What you’ll need- Active Compact/Multi-State license (LPN) (LVN)
- Technical aptitude – Microsoft Office Suite
- Excellent written and verbal communication skills
- Must have active high-speed Wi-Fi
- Must have a home office or HIPAA-compliant workspace
- Plusses:

- Additional Single State licensures
