Targeted Case Manager I
BrightViewTargeted Case Manager I at BrightView Health supporting patients in addiction treatment. Assessing needs, connecting to resources, and ensuring adherence to service plans.
About the role
Key responsibilities & impact- Assess patient eligibility for specific service needs, developing individualized integrated action plans
- Refer and connect patients to resources, monitoring adherence and providing necessary follow-ups
- Complete detailed reports on needs assessment and case management activities
- Provide crisis and de-escalation techniques
- Attend required meetings, including supervision, treatment team meetings, and quarterly in-service
Requirements
What you’ll need- 1 year of experience working with adults in a human service setting
- Bachelor's Degree in a related Behavioral Science field
- Familiarity with MS Office software (Outlook, Teams, Word, etc) preferred
- Familiarity with HIPAA and 42-CFR Part 2 desired
- Prior experience with harm reduction a plus
