About the role

Key responsibilities & impact

Promote and deliver a holistic, client-centered approach to home care, focusing on client needs and rights in line with Consumer Directed Care principles
Contribute to assessments, prevention, early detection, referrals, health promotion, and funding activities
Deliver comprehensive, safe, effective, evidence-based care aligned with current nursing care plans
Ensure compliance with common law, regulatory and state legislation, and organisational policies and procedures
Formulate, review, and case conference care plans as needed
Ensure care plans accurately reflect customer needs and support Aged Care Funding Instrument (ACFI) funding

Requirements

What you’ll need

Bachelor’s Degree required; Nursing, Healthcare is preferred
Any other related discipline or commensurate work experience considered
Minimal experience as a Registered Nurse in a hospital, clinical or residential setting preferred
Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA) Certification is prescribed
Expertise in patient care and medical procedures
Strong knowledge of Clinical Assessment, Medication Administration & Monitoring, Person-Centered Practice, and Quality Care Service Management
Excellent communication, adaptive thinking, planning and prioritisation, and cross-functional team working skills
