About the role

Key responsibilities & impact
  • Promote and deliver a holistic, client-centered approach to home care, focusing on client needs and rights in line with Consumer Directed Care principles
  • Contribute to assessments, prevention, early detection, referrals, health promotion, and funding activities
  • Deliver comprehensive, safe, effective, evidence-based care aligned with current nursing care plans
  • Ensure compliance with common law, regulatory and state legislation, and organisational policies and procedures
  • Formulate, review, and case conference care plans as needed
  • Ensure care plans accurately reflect customer needs and support Aged Care Funding Instrument (ACFI) funding

Requirements

What you’ll need
  • Bachelor’s Degree required; Nursing, Healthcare is preferred
  • Any other related discipline or commensurate work experience considered
  • Minimal experience as a Registered Nurse in a hospital, clinical or residential setting preferred
  • Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA) Certification is prescribed
  • Expertise in patient care and medical procedures
  • Strong knowledge of Clinical Assessment, Medication Administration & Monitoring, Person-Centered Practice, and Quality Care Service Management
  • Excellent communication, adaptive thinking, planning and prioritisation, and cross-functional team working skills
  • Commitment to supporting diversity and inclusion
