About the role

Key responsibilities & impact
  • Conduct comprehensive patient assessments during home visits, including physical, emotional, and psychosocial evaluations.
  • Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and healthcare teams.
  • Manage and monitor symptoms such as pain, nausea, fatigue, and respiratory distress using evidence-based interventions.
  • Provide education and counseling to patients and caregivers regarding disease progression, treatment options, and advance care planning.
  • Coordinate care with physicians, social workers, hospice providers, and other relevant professionals to ensure seamless service delivery.
  • Document all patient interactions, care plans, and clinical outcomes accurately and timely in electronic health records.
  • Respond promptly to urgent patient needs and facilitate appropriate referrals or hospital admissions when necessary.
  • Advocate for patient preferences and support ethical decision-making in end-of-life care scenarios.

Requirements

What you’ll need
  • Current and unrestricted Nurse Practitioner license in the United States.
  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Certification as a Nurse Practitioner (e.g., ANCC or AANP certification).
  • Minimum of 2 years clinical experience, preferably in palliative, hospice, or chronic disease management.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
  • Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification. (preferred)
  • Experience conducting home visits and working within community-based healthcare settings. (preferred)
  • Familiarity with electronic health record (EHR) systems and telehealth technologies. (preferred)
  • Strong background in pain and symptom management for complex patient populations. (preferred)
  • Training in cultural competency and communication skills tailored to diverse patient populations. (preferred)
