Key responsibilities & impact

Conduct comprehensive patient assessments during home visits, including physical, emotional, and psychosocial evaluations.
Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and healthcare teams.
Manage and monitor symptoms such as pain, nausea, fatigue, and respiratory distress using evidence-based interventions.
Provide education and counseling to patients and caregivers regarding disease progression, treatment options, and advance care planning.
Coordinate care with physicians, social workers, hospice providers, and other relevant professionals to ensure seamless service delivery.
Document all patient interactions, care plans, and clinical outcomes accurately and timely in electronic health records.
Respond promptly to urgent patient needs and facilitate appropriate referrals or hospital admissions when necessary.
Advocate for patient preferences and support ethical decision-making in end-of-life care scenarios.

Requirements

What you’ll need

Current and unrestricted Nurse Practitioner license in the United States.
Master’s or Doctoral degree in Nursing from an accredited program.
Certification as a Nurse Practitioner (e.g., ANCC or AANP certification).
Minimum of 2 years clinical experience, preferably in palliative, hospice, or chronic disease management.
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification. (preferred)
Experience conducting home visits and working within community-based healthcare settings. (preferred)
Familiarity with electronic health record (EHR) systems and telehealth technologies. (preferred)
Strong background in pain and symptom management for complex patient populations. (preferred)
Training in cultural competency and communication skills tailored to diverse patient populations. (preferred)
