About the role
Key responsibilities & impact- Conduct brief, structured telehealth intake consultations
- Perform telehealth-based E/M visits or Annual Wellness Visits to establish medical necessity for care navigation services
- Identify high-risk conditions and unmet social needs
- Document findings in Baba’s platform, including accurate coding, SDOH Z-codes, diagnoses, and risk factors
- Develop and approve individualized care plans aligned with patients’ medical treatment plans
- Ensure documentation supports continued medical necessity
- Collaborate with advocates and the clinical operations team to enroll patients safely, compliantly, and compassionately into appropriate care navigation services
Requirements
What you’ll need- M.D. or D.O. with an active, unrestricted U.S. medical license
- Broad multistate licensure strongly required; IMLC participation or 50-state coverage preferred
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
- 2+ years of experience in primary care, telehealth, or care management
- Familiarity with Medicare programs, including care navigation, incident-to requirements, and SDOH documentation
- Proficiency with EMRs, care management platforms, and telehealth workflows
- Bilingual Spanish required
- Legal authorization to work in the United States
