About the role

Key responsibilities & impact

Conduct brief, structured telehealth intake consultations
Perform telehealth-based E/M visits or Annual Wellness Visits to establish medical necessity for care navigation services
Identify high-risk conditions and unmet social needs
Document findings in Baba’s platform, including accurate coding, SDOH Z-codes, diagnoses, and risk factors
Develop and approve individualized care plans aligned with patients’ medical treatment plans
Document continued medical necessity
Collaborate with advocates and the clinical operations team to enroll patients safely, compliantly, and compassionately into appropriate care navigation services

Requirements

What you’ll need

M.D. or D.O. with an active, unrestricted U.S. medical license
Broad multistate licensure strongly required; IMLC participation or 50-state coverage preferred
Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
2+ years of experience in primary care, telehealth, or care management
Familiarity with Medicare programs, including care navigation, incident-to requirements, and SDOH documentation
Proficiency with EMRs, care management platforms, and telehealth workflows
Bilingual Mandarin required
Legal authorization to work in the United States
