About the role
- Care transition management and care provision
- Care coordination and assessment of care needs
- Education of patients, relatives and nursing/home-care services
- Handling of infusion pumps in compliance with the Medical Devices Act (MPG)
- Procedures involving intravenous access (IV lines)
- Supporting continuity and safety of care
- Patient monitoring
- Documentation of services
- Regular feedback to those responsible for therapy/treatment
- Comprehensive and thorough onboarding within an experienced team
Requirements
- Registered nurse (m/f/d) with recognized qualification
- Pediatric nurses (m/f/d) are also welcome to apply
- Strong communication skills
- Empathy
- Ability to work under pressure and a team player
- Valid driver's license
- Basic IT/computer skills
- Willingness to perform day-trip/regional travel assignments
