Virtual Care Case Manager
Austin HealthCase Manager coordinating patient care from hospital to home at Austin Health. Collaborating with healthcare teams to support patients through telehealth and in-home services.
About the role
Key responsibilities & impact- Coordinating patient assessments to provide home nursing services, facilitating video telehealth reviews, and completing complex discharge planning.
- Supporting escalation of the deteriorating patient - both through supporting visiting nurses remotely, and through the provision of on-call support and after-hours escalation.
- Formulating individual plans for patients to support their complex needs, aiming to provide care virtually where possible and supporting the patient and the treating team with a high level of confidence and expertise.
Requirements
What you’ll need- A strong level of clinical experience and confidence, allowing you to make excellent decisions on clinical escalation and care provision.
- A collaborative mindset, working with the treating teams, care partners, the patient and their carer to provide high quality, evidence-based care at home.
- A flexible, person centred approach to care, implementing care plans for patient to allow them to get home as soon as possible.
- A current, unrestricted Driver's license.
