About the role
- Evaluates and provides family-oriented services to home health patients with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments
- Conducts patient and caregiver education, discharge planning, and consultation with other professionals, clinics, or agencies
- Providing initial and ongoing assessment to determine the level of functioning
- Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of patient’s physical and psychosocial functioning
- Developing and revising the plan of care in consultation with the physician and other care-team members
- Responsible for planning, evaluation, and treatment implementation of assigned caseload
- Preparing clinical progress notes
- Supervising therapy assistants and home health aides as appropriate
- Evaluating outcomes
- Planning Discharge
- Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment
- Responsibilities may also include the training of students from university or community colleges assigned
Requirements
- Philadelphia Occupational Therapy (OT) license and CPR Certification
- Demonstrate flexible and efficient time management techniques.
- Strong Clinical & Interpersonal Skills
- Must be able to solve problems independently and confidently by applying analytical and logical thinking.
- Excellent Written & Oral Communication Skills
- Prioritize workload to meet productivity standards and applicable deadlines.
- Communicate with Director of Nursing on a regular basis about project status and issues.
- Establish and maintain positive interpersonal relations with internal and external customers.
- Maintain a positive and respectful attitude and demeanor.
- Report to work on time and prepared to perform duties.
- Able to use standard office equipment and software proficiently.
- Adhere to all agency policies and procedures.
