About the role

Key responsibilities & impact
  • Conduct in-home assessments 3.5 days a week, with some outpatient clinical work, on health plan members
  • Perform remote clinical work 1.5 days a week
  • Provide patient education by matching desired care with best care given
  • Identify diagnoses for care management and active medical management
  • Partner and communicate with the Regional Medical Officer and other team members to develop appropriate care plans
  • Coordinate care with PCPs, specialists, ancillary providers, and other stakeholders
  • Lead a broader clinical team, potentially including nurses, health coaches, social workers, and care coordinators
  • Provide care and support to Alignment Healthcare patients in their homes through the Care Anywhere program
  • Help prevent unnecessary hospitalizations, health complications, and unmanaged disease progression

Requirements

What you’ll need
  • Required: Master’s degree from an accredited NP Program or PA program
  • Active Nurse Practitioner or Physician Assistant California license
  • Active Nurse Practitioner Board Certification/Physician Assistant Certification
  • NPI Number
  • DEA
  • Valid BLS
  • Valid Driver License and current automobile insurance
  • Ability to communicate positively, professionally and effectively with others
  • Effective problem-solving, organizational, and time management skills
  • Ability to work in a fast-paced environment
  • Preferred: One (1) year of prior clinical or home care experience
  • Previous EMR experience preferred
  • Experience caring for older adult (geriatric) patients preferred
  • Ability to regularly stand, walk, sit, use hands and fingers, handle or feel objects, tools, or controls, and reach with hands and arms
  • Ability to frequently lift and/or move up to 20 pounds
