About the role
Key responsibilities & impact- Conduct in-home assessments 3.5 days a week, with some outpatient clinical work, on health plan members
- Perform remote clinical work 1.5 days a week
- Provide patient education by matching desired care with best care given
- Identify diagnoses for care management and active medical management
- Partner and communicate with the Regional Medical Officer and other team members to develop appropriate care plans
- Coordinate care with PCPs, specialists, ancillary providers, and other stakeholders
- Lead a broader clinical team, potentially including nurses, health coaches, social workers, and care coordinators
- Provide care and support to Alignment Healthcare patients in their homes through the Care Anywhere program
- Help prevent unnecessary hospitalizations, health complications, and unmanaged disease progression
Requirements
What you’ll need- Required: Master’s degree from an accredited NP Program or PA program
- Active Nurse Practitioner or Physician Assistant California license
- Active Nurse Practitioner Board Certification/Physician Assistant Certification
- NPI Number
- DEA
- Valid BLS
- Valid Driver License and current automobile insurance
- Ability to communicate positively, professionally and effectively with others
- Effective problem-solving, organizational, and time management skills
- Ability to work in a fast-paced environment
- Preferred: One (1) year of prior clinical or home care experience
- Previous EMR experience preferred
- Experience caring for older adult (geriatric) patients preferred
- Ability to regularly stand, walk, sit, use hands and fingers, handle or feel objects, tools, or controls, and reach with hands and arms
- Ability to frequently lift and/or move up to 20 pounds
