About the role

Key responsibilities & impact

Conduct in-home assessments 3.5 days a week, with some outpatient clinical work, on health plan members
Perform remote clinical work 1.5 days a week
Provide patient education by matching desired care with best care given
Identify diagnoses for care management and active medical management
Partner and communicate with the Regional Medical Officer and other team members to develop appropriate care plans
Coordinate care with PCPs, specialists, ancillary providers, and other stakeholders
Lead a broader clinical team, potentially including nurses, health coaches, social workers, and care coordinators
Provide care and support to Alignment Healthcare patients in their homes through the Care Anywhere program
Help prevent unnecessary hospitalizations, health complications, and unmanaged disease progression

Requirements

What you’ll need

Required: Master’s degree from an accredited NP Program or PA program
Active Nurse Practitioner or Physician Assistant California license
Active Nurse Practitioner Board Certification/Physician Assistant Certification
NPI Number
DEA
Valid BLS
Valid Driver License and current automobile insurance
Ability to communicate positively, professionally and effectively with others
Effective problem-solving, organizational, and time management skills
Ability to work in a fast-paced environment
Preferred: One (1) year of prior clinical or home care experience
Previous EMR experience preferred
Experience caring for older adult (geriatric) patients preferred
Ability to regularly stand, walk, sit, use hands and fingers, handle or feel objects, tools, or controls, and reach with hands and arms
Ability to frequently lift and/or move up to 20 pounds
