About the role
Key responsibilities & impact- Conduct in-home assessments 3.5 days per week, with some outpatient clinical work
- Perform remote work 1.5 days per week
- Provide patient education by matching desired care with best care given
- Identify diagnoses for care management and active medical management
- Partner with the Regional Medical Officer and other team members to develop appropriate care plans
- Coordinate care with PCPs, specialists, and ancillary providers
- Lead a broader clinical team that may include nurses, health coaches, social workers, and care coordinators
- Provide care and support to eligible Alignment Healthcare patients in their homes
- Help prevent unnecessary hospitalizations, health complications, and unmanaged disease progression
Requirements
What you’ll need- Master’s degree from an accredited NP Program or PA program
- Active Nurse Practitioner or Physician Assistant California license
- Active Nurse Practitioner Board Certification/Physician Assistant Certification
- NPI Number
- DEA
- Valid BLS
- Valid Driver License and current automobile insurance
- Ability to communicate positively, professionally and effectively with others
- Effective problem-solving, organizational, and time-management skills
- Ability to work in a fast-paced environment
- One year of prior clinical or home care experience preferred
- Previous EMR experience preferred
- Experience caring for older adult (geriatric) patients preferred
- Ability to perform in-home visits and meet the stated physical requirements, including frequently lifting and/or moving up to 20 pounds
