About the role

Key responsibilities & impact

Conduct in-home assessments 3.5 days per week, with some outpatient clinical work
Perform remote work 1.5 days per week
Provide patient education by matching desired care with best care given
Identify diagnoses for care management and active medical management
Partner with the Regional Medical Officer and other team members to develop appropriate care plans
Coordinate care with PCPs, specialists, and ancillary providers
Lead a broader clinical team that may include nurses, health coaches, social workers, and care coordinators
Provide care and support to eligible Alignment Healthcare patients in their homes
Help prevent unnecessary hospitalizations, health complications, and unmanaged disease progression

Requirements

What you’ll need

Master’s degree from an accredited NP Program or PA program
Active Nurse Practitioner or Physician Assistant California license
Active Nurse Practitioner Board Certification/Physician Assistant Certification
NPI Number
DEA
Valid BLS
Valid Driver License and current automobile insurance
Ability to communicate positively, professionally and effectively with others
Effective problem-solving, organizational, and time-management skills
Ability to work in a fast-paced environment
One year of prior clinical or home care experience preferred
Previous EMR experience preferred
Experience caring for older adult (geriatric) patients preferred
Ability to perform in-home visits and meet the stated physical requirements, including frequently lifting and/or moving up to 20 pounds
