About the role

Key responsibilities & impact
  • Conduct in-home assessments 3.5 days per week, with some outpatient clinical work
  • Perform remote work 1.5 days per week
  • Provide patient education by matching desired care with best care given
  • Identify diagnoses for care management and active medical management
  • Partner with the Regional Medical Officer and other team members to develop appropriate care plans
  • Coordinate care with PCPs, specialists, and ancillary providers
  • Lead a broader clinical team that may include nurses, health coaches, social workers, and care coordinators
  • Provide care and support to eligible Alignment Healthcare patients in their homes
  • Help prevent unnecessary hospitalizations, health complications, and unmanaged disease progression

Requirements

What you’ll need
  • Master’s degree from an accredited NP Program or PA program
  • Active Nurse Practitioner or Physician Assistant California license
  • Active Nurse Practitioner Board Certification/Physician Assistant Certification
  • NPI Number
  • DEA
  • Valid BLS
  • Valid Driver License and current automobile insurance
  • Ability to communicate positively, professionally and effectively with others
  • Effective problem-solving, organizational, and time-management skills
  • Ability to work in a fast-paced environment
  • One year of prior clinical or home care experience preferred
  • Previous EMR experience preferred
  • Experience caring for older adult (geriatric) patients preferred
  • Ability to perform in-home visits and meet the stated physical requirements, including frequently lifting and/or moving up to 20 pounds
