About the role

Key responsibilities & impact
  • Prepare and present in-service presentations to referral sources and follow up with them
  • Educate referral sources on Home Health service components
  • Serve as a liaison between Home Health and physicians, hospitals, long-term care facilities, assisted living facilities, and other referral sources
  • Communicate frequently with referral sources to ensure expectations and patient needs are met
  • Identify and resolve issues, dissatisfaction, or quality-of-care problems reported by referral sources
  • Oversee and monitor primary relationships for consistency and quality
  • Submit timely written documentation of contacts, referrals, and contracts
  • Assist in developing new programs to meet community needs
  • Establish relationships with physicians, hospitals, long-term care facilities, and assisted living facilities

Requirements

What you’ll need
  • 3–5 years of Hospice / Home Health experience
  • Proven success reaching or exceeding quotas
  • Excellent organizational, communication, and interpersonal skills
  • Strategic thinking ability to evolve marketing calendars and processes
  • Self-starter with a strong sense of urgency and initiative
  • Ability to multitask in a fast-paced environment
  • Ability to work well with multiple teams and partners
  • Solid computer skills, including Excel, Word, and PowerPoint
  • Positive collaborator able to balance initiative and direction
  • Ability to work collaboratively with sales, marketing, operations, and other cross-functional teams
