About the role
Key responsibilities & impact- Prepare and present in-service presentations to referral sources and follow up with them
- Educate referral sources on Home Health service components
- Serve as a liaison between Home Health and physicians, hospitals, long-term care facilities, assisted living facilities, and other referral sources
- Communicate frequently with referral sources to ensure expectations and patient needs are met
- Identify and resolve issues, dissatisfaction, or quality-of-care problems reported by referral sources
- Oversee and monitor primary relationships for consistency and quality
- Submit timely written documentation of contacts, referrals, and contracts
- Assist in developing new programs to meet community needs
- Establish relationships with physicians, hospitals, long-term care facilities, and assisted living facilities
Requirements
What you’ll need- 3–5 years of Hospice / Home Health experience
- Proven success reaching or exceeding quotas
- Excellent organizational, communication, and interpersonal skills
- Strategic thinking ability to evolve marketing calendars and processes
- Self-starter with a strong sense of urgency and initiative
- Ability to multitask in a fast-paced environment
- Ability to work well with multiple teams and partners
- Solid computer skills, including Excel, Word, and PowerPoint
- Positive collaborator able to balance initiative and direction
- Ability to work collaboratively with sales, marketing, operations, and other cross-functional teams
