About the role

Key responsibilities & impact
  • Provide skilled physical therapy services in accordance with physician orders and individualized plans of care
  • Evaluate patients and develop personalized treatment programs to improve function and quality of life
  • Assess mobility, strength, balance, and safety needs
  • Document patient evaluations, treatments, and progress in a timely and accurate manner
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes
  • Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals
  • Supervise Physical Therapist Assistants as appropriate
  • Manage schedules and provide one-on-one care in patients’ homes
  • Help individuals regain strength, mobility, and independence

Requirements

What you’ll need
  • Graduate of an accredited Physical Therapy program
  • Current Physical Therapist license in Pennsylvania
  • Current CPR certification (or ability to obtain upon hire)
  • Valid driver's license
  • Reliable transportation
  • Proof of auto insurance
  • Strong communication, organizational, and documentation skills
  • Home health experience preferred, but not required
