About the role
Key responsibilities & impact- Provide skilled physical therapy services in accordance with physician orders and individualized plans of care
- Evaluate patients and develop personalized treatment programs to improve function and quality of life
- Assess mobility, strength, balance, and safety needs
- Document patient evaluations, treatments, and progress in a timely and accurate manner
- Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes
- Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals
- Supervise Physical Therapist Assistants as appropriate
- Manage schedules and provide one-on-one care in patients’ homes
- Help individuals regain strength, mobility, and independence
Requirements
What you’ll need- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in Pennsylvania
- Current CPR certification (or ability to obtain upon hire)
- Valid driver's license
- Reliable transportation
- Proof of auto insurance
- Strong communication, organizational, and documentation skills
- Home health experience preferred, but not required
