About the role
Key responsibilities & impact- Conducting Start of Care comprehensive assessments
- Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
- Preparing accurate, orderly, and timely documentation
- Coordinating care with patient and provide proper reports to Case Manager
- Provides health care instructions to the patient/family as appropriate per assessment and plan
Requirements
What you’ll need- An active RN license in the state in which practicing (Minimum of 1 year of experience preferred)
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Compassionate and empathetic approach to patient care
- Ability to work independently and as part of a team
- Knowledge of hospice regulations and standards of care
- Reliable transportation and willingness to travel.
