About the role
Key responsibilities & impact- Provide skilled nursing care to hospice patients in their homes or in a hospice facility.
- Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
- Administer medication, monitor vital signs, and manage symptoms to ensure patient comfort.
- Educate and support patients and their families regarding end-of-life care and symptom management.
- Coordinate care with other healthcare professionals and community resources.
- Offer emotional support, counseling, and education to patients and their families, addressing their unique needs and concerns.
Requirements
What you’ll need- You should have an active RN license in the state in which you are practicing or be a new grad with eligibility to take NCLEX.
- Minimum 1 year of experience in hospice, palliative care, or related field preferred but not required.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work collaboratively in a multidisciplinary team.
- Compassionate and empathetic approach to patient care.
- Knowledge of hospice regulations and practices preferred.
