About the role

Key responsibilities & impact
  • Provide skilled nursing care to hospice patients in their homes or in a hospice facility
  • Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
  • Administer medication, monitor vital signs, and manage symptoms to ensure patient comfort
  • Educate and support patients and their families regarding end-of-life care and symptom management
  • Coordinate care with other healthcare professionals and community resources
  • Offer emotional support, counseling, and education to patients and their families, addressing their unique needs and concerns

Requirements

What you’ll need
  • active RN license in the state in which you are practicing or be a new grad with eligibility to take NCLEX
  • Minimum 1 year of experience in hospice, palliative care, or related field preferred but not required
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Ability to work collaboratively in a multidisciplinary team
  • Compassionate and empathetic approach to patient care
  • Knowledge of hospice regulations and practices preferred.
