Coordinate and schedule virtual appointments with healthcare providers and specialists
Act as a liaison between clients, their families, and healthcare providers to ensure effective communication and coordination of care
Provide virtual support and education to clients and their families on health-related topics and resources
Monitor clients' health status and report any changes or concerns to the appropriate healthcare professionals
Maintain accurate and up-to-date client records and documentation
Collaborate with our team of in-home caregivers to ensure that clients' care plans are being followed and any issues are addressed promptly
Continuously strive to improve the quality of care and services provided to our clients.

Requirements

What you’ll need

High school diploma or equivalent, with a minimum of 2 years of experience in a healthcare setting
Certified Medical Assistant (CMA) or equivalent certification preferred
Experience with virtual health assessments and consultations
Excellent communication and interpersonal skills
Strong organizational and time-management skills
Proficiency in using virtual communication platforms and electronic medical records
Compassionate, patient, and empathetic demeanor
Ability to work independently and as part of a team
Must be able to pass a background check and drug screening.
