About the role

Key responsibilities & impact
  • Conduct comprehensive assessments of clients' health care needs and develop individualized care plans
  • Provide ongoing monitoring and evaluation of clients' health status and make necessary adjustments to care plans

  • Utilize telehealth technology to conduct virtual consultations with clients and their families

  • Participate in team meetings and trainings to share knowledge and expertise with colleagues

Requirements

What you’ll need
  • Active nursing license in good standing
  • Minimum of 3 years of experience in a clinical setting, preferably with a focus on senior care
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage a caseload effectively
  • Proficiency with technology and ability to conduct virtual consultations
  • Compassionate and empathetic attitude towards seniors and their families
  • Willingness to travel for occasional in-person visits with clients (if needed)
  • Availability to work full-time, Monday-Friday during regular business hours
