Overnight Residential Counselor

• Provides direct support to people with intellectual and developmental disabilities in a small group residential setting or an individual living arrangement.
• Works together with the people we serve, their families, friends, employers, and other support staff to transform their lives by either building independence skills or increasing each person's quality of life.
• Administers medication under the Medication Administration Plan (MAC), implements treatment plans, and assists people with activities of daily living, such as household chores and cooking.
• Strengthens residents' connection to their community by providing transportation on outings such as shopping and doctor's appointments.
• Develops, implements, and documents treatment, including ISP/IPs and clinical programs.
• Provides training and support to persons served, including rehabilitative interventions, behavioral interventions and monitoring, fire & safety regulations, appointment scheduling, money management, ADLs, grocery shopping, meal planning and cooking, household chores and other related areas.
• Plans recreational and social activities to promote the emotional well-being of persons served and improve quality of life.
• Assesses residents’ physical and emotional status, and reports changes to supervisor.
• Maintains records for persons served and communicates with interested parties such as program nurse, funding agency, case managers, other service providers, Visiting Nurses Association nurses, and physicians.
• Participates in all mandated training and staff meetings as directed by manager.

Residential Counselor

• Assists in the implementation of individual’s IP individualized day non-vocational (IDN) goals
• Provides support and guidance to persons served in their residential home and in the community
• Engages and supports individuals in achieving the goals and assist with any emotional or behavioral supports
• Documents all services daily
• Works directly with persons served individually and develops skills for persons served
• Implements prescribed clinical, educational, behavioral programs
• Responsible for specific data collection and performing bi-monthly analysis of data
• Maintains physical upkeep of the program area
• Acts as advocate for persons served
• Completes required direct care training

IDN Counselor – Residential

• Perform active rescues when texter safety has been identified as imminent risk.
• Monitor volunteer Crisis Counselor conversations for training adherence and quality support.
• Work with volunteer Crisis Counselors in providing coping skills and exercise safety protocols in high-risk situations.
• Assist volunteer Crisis Counselors to identify texter needs and develop a support and plan for safety.
• Ensure volunteer Crisis Counselors adherence to code of conduct and escalate if required.
• Provide real-time feedback on conversation quality.
• Participate in trainings and internal working groups to increase quality service delivery.

Crisis Counselor Supervisor

• Provide direct support to people with intellectual and developmental disabilities in a small group residential setting or an individual living arrangement.

• Manage PGT-M cases from initiation through test completion.

Laboratory Genetic Counselor

• Under general supervision, coordinates and performs a variety of administrative, registration, and financial support duties for the unit/department.

This includes rehabilitative interventions, behavioral interventions and monitoring, fire & safety regulations, appointment scheduling, money management, ADLs, grocery shopping, meal planning and cooking, household chores and other related areas. Perform hands-on tasks in the program as required in order to facilitate implementation of support plans.
• Assists individuals in becoming fully integrated into their communities. Assists residents with community-based support and resource services. Plans recreational and social activities to promote the emotional well-being of persons served and improve quality of life. This includes holiday and birthday parties, celebrations, outings, community travel, church activities, decorating, shopping and other related activities.
• Provides transportation, as necessary.
• Assesses residents’ physical and emotional status, and report changes to supervisor.
• Monitor residents’ funds, as needed.
• Maintains records for persons served. Collects behavior and academic data and maintains progress notes. Enters information into electronic health record. Documents all information related to incidents, programs and routine activities in a communication log.
