Community Health Worker

• Serves as a key connector between underserved communities and health/social service systems
• Focuses on promoting health equity and improving care coordination
• Responsible for outreach, community member or patient advocacy and care coordination
• Builds trusting relationships to mediate between individuals and health/social service systems
• Identifies and engages individuals and families through outreach activities and community events
• Evaluates the specific needs and challenges faced by community members
• Identifies barriers such as housing and food insecurity and connects individuals to appropriate resources
• Applies specialized knowledge to provide targeted education and resource navigation
• May conduct outreach activities, including home visits
• Maintains accurate and up-to-date records to support advocacy efforts
