Advanced Practice Provider

• Provide outpatient general cardiology care for established patients in Mitchell and surrounding outreach communities
• Assess and manage recheck patients
• Support North Central Heart cardiologists during Mitchell outreach clinics
• Independently conduct follow-up outreach visits after an NCH physician has established the patient treatment plan
• Review diagnostic results
• Provide patient education
• Collaborate with NCH cardiologists to promote safe, coordinated, patient-centered cardiovascular care
