Community Manager

• Provide services to youth and families with complex behavioral health needs
• Serve a limited number of youth and families at a time
• Appointments take place in the home or community
• Deliver intensive home-based case management services in accordance with state mandates and agency policies
• Collaborate with clinical therapists to support individualized family service plans
• Document all services provided and maintain accurate client records in the electronic health record system
• Participate in 24/7 on-call rotation as required
