About the role

  • Engage with clients within required timelines based on their assigned risk level and referral protocol.
  • Collaborate with clients and referral agents to clarify counselling goals and expectations.
  • Conduct comprehensive assessments and develop individualized treatment plans.
  • Deliver short- to medium-term counselling to support goal attainment and client well-being.
  • Provide ongoing clinical support in-person, virtually, or by phone (as appropriate and based on training).
  • Act as a case manager—coordinating care, making referrals, and advocating on behalf of clients.
  • Maintain open communication with referral agents and secure client consent as needed.
  • Track client progress, satisfaction, and service outcomes.
  • Consult regularly with the Clinical Director/Supervisor and peers.
  • Respond to inquiries from referral agents and clients, and keep stakeholders informed of case developments.
  • Follow standardized case booking and confidentiality protocols.
  • Ensure timely documentation and case closure.
  • Meet or exceed service delivery expectations for billable hours and case management.
  • Represent and promote TELUS Health’s values and services both internally and externally.
  • Adhere to all organizational policies, procedures, and clinical standards.

Requirements

  • Master’s degree in Social Work, Counselling, Psychology, or related field
  • Registration in good standing with: Ontario College of Social Workers and Social Service Workers (OCSWSSW), or College of Registered Psychotherapists of Ontario (CRPO)
  • Minimum two (2) years of post-graduate clinical experience
  • Professional liability insurance
  • Availability during standard business hours
  • Proven ability to support individuals with complex challenges such as: Mental health and addiction concerns, Social exclusion and homelessness, Domestic violence and anger management, Anti-criminal thinking and sexual offenses, Justice system involvement
  • Proficiency in substance use assessment and treatment
  • Strong knowledge of local community resources and experience working with diverse populations
  • Experience with mandated clients is considered a strong asset.
