About the role
- Advocates for the patient and family throughout the entire episode of care
- Monitors patient’s progression intervening as needed to ensure efficient patient progress
- Coordinates all aspects of discharge planning and establishes a safe discharge plan
Requirements
- BSN
- 2 years of medical surgical nursing experience
- 1-2 years of acute care, homecare or population health RN case management experience (preferred)
- CCM certification (preferred)
