About the role
- Serve as the clinical leader and trusted advisor to physicians and partner practices in the Northeast market (NY, CT, VT).
- Dyad Partner with physician engagement teams to build and strengthen relationships across independent and group practices.
- Serve as a visible clinical champion for value-based care and population health initiatives making routine and regular visits to partner practices.
- Engage with Post Acute and Acute facilities to assist with establishing physician engagement and value-based care management programs.
- Educate and influence physicians and practice leaders on evidence-based care models, risk adjustment, quality improvement, and care coordination strategies.
- Translate organizational goals into practical clinical workflows that support improved outcomes and performance under value-based contracts.
- Monitor market performance against quality, utilization, and financial targets, and collaborate with practices to identify and close performance gaps.
- Provide expert perspective and develop clinical leader relationships with partner health plans.
- Execute on model of care medical leadership responsibilities, including case reviews, interdisciplinary care team meetings, clinical escalations.
- Innovate on care model delivery in partnership with care navigation, health plan partners, and other interdisciplinary teams.
- Partner with operational and analytics teams to identify opportunities for improved risk documentation, care management, and patient engagement.
- Provide clinical insights and guidance to support change management initiatives at the practice level.
- Help design and implement market-specific strategies to drive adoption of best practices.
- Represent the organization in market-facing forums, physician meetings, and clinical advisory groups.
- Provide feedback from the field to inform organizational strategy and program development.
- Foster a culture of collaboration, innovation, and accountability within the market.
Requirements
- Experience in a physician leadership role with responsibility for influencing practice behavior and clinical outcomes.
- Advanced knowledge of value-based care, risk adjustment, population health, and quality improvement strategies.
- Prior experience working in a population health, accountable care, or managed services environment.
- Strong communication, interpersonal, and relationship-building skills.
- Ability to travel regularly within the Northeast market.
- Background in working with both independent practices and larger health system groups.
- Proven ability to drive physician engagement and adoption of new clinical models.
