• Prepares and presents presentations of various in-services to referral sources and follows-up with the referral sources.
• Educates referral sources on Home Health services.
• Serves as a liaison between Home Health and various facilities.
• Communicates frequently with referral sources to ensure patient needs are met.
• Assists in identifying and resolving any issues experienced by the referral source.
• Monitors primary relationships for consistency and quality.
• Submits written documentation of all contacts, referrals, and contracts.
• Assists in developing new programs to meet community needs.
